How Truvada and Descovy Work

Both drugs block a step HIV needs to copy itself inside your body. Stop that step early enough and HIV cannot take hold, even after an exposure.

Truvada contains two active ingredients: tenofovir disoproxil fumarate, known as TDF, and emtricitabine, known as FTC. Descovy also contains FTC, but pairs it with a newer form of tenofovir called tenofovir alafenamide, or TAF. TAF delivers the active drug more efficiently, which means a smaller amount reaches the kidneys and bones. That is where the safety difference comes from. Both drugs are equally effective at preventing HIV. The difference is tolerability, not efficacy.

Kidney and Bone Safety — Truvada vs Descovy for Long-Term Use

This is the question our long-term expats and older patients ask most often. The short answer is that Descovy is easier on your kidneys and bones over time.

A large clinical trial called DISCOVER compared TDF-based Truvada with TAF-based Descovy over two years. Descovy users showed less decline in kidney filtration and less loss of bone density. For most healthy adults under 50 with normal kidney function, the practical difference is modest. But if you already have reduced kidney function, early bone loss, or you plan to be on PrEP for many years, Descovy is worth a conversation.

Side Effects Compared

Neither drug causes serious problems in most people. The first few weeks on Truvada can bring mild nausea, headaches, or an unsettled stomach. These usually pass on their own.

Descovy has a slightly different pattern. Some patients notice modest weight gain and small changes in cholesterol over time. This is worth monitoring but rarely a reason to switch. If you have a history of high cholesterol, mention it at your consultation.

Neither drug interacts badly with alcohol. Neither will affect your time in Pattaya in any meaningful way. Take your tablet consistently and you are covered.

Who Should Not Take Descovy — The Approval Gap

This point matters, and I want to be direct about it. Descovy is not approved for people assigned female at birth who are at risk of HIV through vaginal sex. It was not adequately studied in that group, so regulatory bodies including the FDA have not cleared it for that use.

If you are a woman, or a transgender man who has not had certain surgeries, and your primary HIV risk is through vaginal sex, Truvada or its generic equivalent is your option. Full stop.

Daily PrEP vs On-Demand PrEP — Only One Drug Qualifies

A lot of tourists and short-stay visitors ask about on-demand PrEP. Instead of a pill every day, you take it around a specific encounter. This is called the 2-1-1 method.

You take two tablets two to twenty-four hours before sex, one tablet twenty-four hours after, and one more tablet twenty-four hours after that. It is well-supported by clinical evidence for men who have sex with men.

Here is the key point: only Truvada, the TDF-based version, is supported for on-demand dosing. Descovy has not been studied for this and is not approved for it. Truvada vs Descovy Cost in Pattaya

You cannot start PrEP safely without a few baseline tests. This is not optional.

First, a confirmed negative HIV test. PrEP given to someone already living with HIV can cause the virus to develop drug resistance, making it much harder to treat later.
