When symptoms do show up, women usually notice a frothy, yellow-green discharge with a strong smell, along with itching, redness, or a burning feeling during sex or urination. Men more often have no symptoms, though some notice a mild discharge or irritation at the tip of the penis.

Because symptoms are often absent or easy to dismiss, many people only find out through a routine STI screen or when a partner tests positive. If you have had unprotected sex recently and something feels off, get tested. Do not sit on it.

How Is Trichomoniasis Diagnosed?

Metronidazole has a well-documented interaction with alcohol that can cause flushing, nausea, and vomiting. Phase 3 clinical data for secnidazole does not show the same established reaction, and in practice it is generally considered to carry a less strict alcohol restriction.

That said, standard prescribing guidance still recommends avoiding alcohol for 48 hours after your dose as a precaution. You are already dealing with something your body is fighting. If symptoms are still present more than a week after treatment, come back in. Do not take another dose on your own.

Resistance to this class of drug, where the parasite does not respond normally, is documented and appears to be increasing in some populations. It is not common, but it does happen. Recurrence is almost always down to reinfection from an untreated partner rather than true resistance. This is why partner treatment is a requirement, not a recommendation.

Should You Test for Other STIs at the Same Time?

Trichomoniasis frequently shows up alongside other infections. Chlamydia, gonorrhoea, and bacterial vaginosis are common ones to find at the same time. Treating trichomoniasis does not rule out anything else.

What to Do After Taking Secnidazole

Avoid sex, including oral sex, for at least seven days after your dose and until your partner has also completed treatment. This is standard guidance and it is there for a reason.

Symptoms should start to improve within a few days. If things are not clearly better by day seven, come back in rather than waiting it out.
