Why Asymptomatic Chlamydia Is Far More Common Than People Expect
When patients come into Doctor Pattaya after unprotected sex, the first thing most of them say is "but I don’t have any symptoms." That is not reassuring. That is the normal picture with this infection.
The majority of women with chlamydia have no symptoms at all. A large proportion of men do not either. No discharge, no burning, no pelvic pain, nothing. The body simply does not produce a noticeable response, and the bacteria continue to multiply on the lining of the urethra, cervix, rectum, or throat.
This is why you cannot rely on your body to tell you something is wrong. It will not.
The Difference Between "Dormant" and Asymptomatic Chlamydia
A lot of patients use the word "dormant" to describe chlamydia that has been sitting undetected for a long time. It is understandable, but it is not accurate.
Dormant would mean the bacteria are inactive. That is not what happens. Asymptomatic chlamydia is active. The bacteria are replicating, you can pass it to a partner, and in some people, tissue damage is already underway, all without a single symptom you can feel.
This is why waiting to see if symptoms develop is not a safe approach.
Where Chlamydia Hides Beyond the Genitals
This is one of the most under-discussed parts of chlamydia, and it is especially relevant in Pattaya where people engage in a wide range of sexual activity.
Rectal chlamydia is almost always completely silent. No pain, no discharge, no indication at all. It can occur in anyone who has had receptive anal sex, including women, and it will not show up on a urine test. A rectal swab is needed. Throat chlamydia from oral sex also tends to produce no symptoms and also requires a specific swab to detect.

The Silent Damage Happening While You Feel Fine
This is the part that concerns me most when a patient has been carrying an undiagnosed infection for weeks or months.
In women, untreated chlamydia can spread into the uterus and fallopian tubes, causing pelvic inflammatory disease, or PID. PID often develops without symptoms, and repeated or prolonged infection leads to scarring of the fallopian tubes. That scarring is a leading cause of infertility and significantly raises the risk of ectopic pregnancy, where a fertilised egg implants in the tube rather than the uterus. An ectopic pregnancy is a medical emergency.
In men, the bacteria can spread to the epididymis, a small tube behind the testicle. This can affect fertility and cause lasting discomfort. There is also a well-documented link between untreated chlamydia and a higher risk of acquiring HIV if exposed, because genital inflammation makes it easier for the virus to enter the body.
A small number of people also develop reactive arthritis after chlamydia, with joint pain appearing weeks later, often long after the original infection has been forgotten.
Can Asymptomatic Chlamydia Clear on Its Own?
This question comes up constantly. The honest answer is: very rarely, and you should not count on it.
Spontaneous clearance does occasionally happen, but there is no way to know whether your body will manage it. During any period of waiting, the infection remains transmissible and silent damage continues.
Antibiotic treatment is fast and effective. A short course of doxycycline or a single dose of azithromycin clears the infection in the vast majority of cases. There is no good reason to wait once a diagnosis is confirmed.
Who Should Be Getting Tested for Asymptomatic Chlamydia in Pattaya
We see a very specific mix of patients here. Some are tourists who had unprotected sex and are flying home in a few days. Some are long-term expats who last got tested a long time ago. Some are people who had one encounter they are not sure about. All of them are in the right place.
Test if you have had unprotected vaginal, anal, or oral sex with a new or casual partner and have not been screened since. Test if your last check was more than a year ago and you have been sexually active. Test before starting a new relationship. And test before you fly home, because treating something here is far simpler than explaining it to a doctor back home who does not know your history.
The window period for chlamydia testing is typically one to two weeks from exposure. Testing too early can produce a false negative even with an active infection. If your exposure was in the last week, come in anyway. We will advise you on timing and arrange a follow-up if needed.

If your test comes back positive, any recent sexual partner should know so they can get tested and treated. You do not need to have been in a relationship for this to apply.
