Symptoms to Watch For — and Why You Might Have None

The most common signs in men are a burning sensation when urinating and a white, yellow, or green discharge. Women often get a burning feeling too, along with increased vaginal discharge or pelvic discomfort.

Here is the part most people miss. A large number of people with gonorrhea, particularly women, have no symptoms at all. They feel completely fine. That is exactly why testing matters after any unprotected encounter, not just when something feels wrong. A urine test or swab tells us quickly whether we need to act.

Where Gonorrhea Can Infect — Beyond the Obvious Sites

Most people think of gonorrhea as a genital infection. It can also infect the throat and rectum, and the throat form is usually completely silent. No sore throat. No warning signs. You would not know it is there unless we swab for it.

If there is any chance of oral or anal exposure, tell us when you come in. We swab the relevant sites. Missing an infection at one site while treating another means it can persist and spread further. It takes thirty seconds and it matters.

How We Diagnose It at the Clinic

We run a highly accurate urine test or swab, depending on which sites may be affected. For most men with symptoms, a urine sample is enough. For throat or rectal exposure, we take swabs from those sites.

You do not need to prepare anything beforehand. Walk in, tell us what happened, and we take it from there. In clear symptomatic cases, we can start treatment at the same visit without waiting for results, then confirm everything once they come back.

The current standard treatment is a single intramuscular injection of ceftriaxone. It goes into the muscle, usually the upper outer thigh or buttock. It works faster and more reliably than oral antibiotics, and it removes any risk of missing a dose or stopping early.

Because gonorrhea and chlamydia frequently occur together, we routinely add a second antibiotic to cover both at once. That is usually azithromycin as a single oral dose, or doxycycline taken over seven days. Which one you get depends on your full picture: any allergies, your symptoms, and what else we are testing for.

You leave the clinic having treated both infections in one visit. No need to come back for a second round. This combination approach follows WHO-recommended guidelines, and it is what we use at Doctor Pattaya.

How Fast Symptoms Clear After Treatment

Discharge and pain when urinating typically start to improve within two to three days of the injection. By day five to seven, most patients are symptom-free.

Feeling better does not mean you are cleared to have sex again. The standard advice is to abstain for seven full days after treatment, and until any partner has also been treated. Having sex before that window closes risks reinfecting yourself even if your own treatment worked.

A follow-up test at least fourteen days after treatment is recommended to confirm everything has cleared. For throat infections especially, we prefer to verify rather than assume. If you are still in Pattaya, come back for that. If you are heading home before the two weeks are up, we can advise on getting follow-up done through your GP.

Antibiotic Resistance — Why the Pharmacy Is Not the Answer

Thailand’s pharmacies will sell you antibiotics without a prescription. A lot of people try to handle it that way to avoid the clinic conversation. The instinct makes sense, but here is why it backfires.

Gonorrhea has developed resistance to several antibiotics that were once standard, including older fluoroquinolones and azithromycin used alone. Without a proper assessment, the antibiotic you buy off the shelf may suppress your symptoms for a week while the infection stays active and continues to do damage.

Self-treating also means you almost certainly miss any co-existing infection. Chlamydia, syphilis, and gonorrhea can all be present at the same time. A pharmacist does not check for that. We do.

Partner Treatment — Why This Part Is Not Optional

Reinfection from an untreated partner is the most common reason gonorrhea comes back after treatment. If your partner is still in Pattaya, bring them in or have them come separately. We can test and treat both of you on the same day.

If your partner has already left Thailand, that is a harder conversation, but not one to skip. We can discuss what options are available depending on your situation. The important thing is that both of you are treated before resuming sexual contact.

We collect a urine sample or swabs depending on your exposure sites. In symptomatic cases, we administer the ceftriaxone injection at the same visit. The injection takes less than a minute. After Treatment — Retesting, Safe Sex, and Staying Clear

Abstain for seven days after treatment. Both of you. That is the rule.

Get a follow-up test at least two weeks after treatment, especially if you had a throat infection or symptoms that linger past day seven. Symptoms that do not improve by day three to five are worth a call to us, since persistent symptoms can occasionally point to a resistant strain that needs a different approach.

We also recommend a full STD panel at the same time as gonorrhea treatment, or shortly after. One session covers gonorrhea, chlamydia, syphilis, HIV, hepatitis B and C, and other infections depending on your situation. If you are sexually active and in Pattaya, this is just good practice.

For HIV specifically: if you had a high-risk exposure within the last seventy-two hours, ask us about PEP today. Do not wait on that one. Doxycycline-PEP for bacterial STIs is also something we can discuss if it is appropriate for your situation.
