The problem is that none of this feels like anything for a very long time.

The window period for hepatitis C antibody testing is roughly 8 to 11 weeks after exposure. If you have had a recent exposure and cannot wait that long, an HCV RNA PCR test can detect the virus sooner, typically within a few weeks of infection.

Getting Tested: How Hepatitis C Is Diagnosed in Pattaya

Testing starts with an anti-HCV antibody test. This tells us whether your immune system has ever encountered the hepatitis C virus. A positive antibody result does not automatically mean you have an active infection, so we do not stop there.

The next step is an HCV RNA PCR test, which looks for the virus itself in your blood. This confirms whether the infection is active and needs treatment. If it comes back positive, we also run genotyping to identify which strain of the virus you have. That matters because it shapes what we prescribe and how long you take it.

Which Hepatitis C Treatment Will You Be Prescribed

Two regimens are commonly used. Sofosbuvir/velpatasvir works against all known hepatitis C genotypes and is typically prescribed for 12 weeks. Glecaprevir/pibrentasvir is another option that can run for as little as 8 weeks in the right patient. That shorter course is the one people ask about most, and it is a real option if you have never been treated before and do not have cirrhosis.

What the Treatment Feels Like Day to Day

This is what most patients genuinely want to know, especially tourists who are mid-holiday and not keen on being sidelined for two months.

For the vast majority of patients, the answer is reassuring. Modern DAAs are very well tolerated. Some people notice mild fatigue or an occasional headache in the first week or two. That is about as bad as it gets for most people. This is completely different from the old interferon-based treatment, which caused severe flu-like reactions and required weekly injections. Those days are over.

I do ask all my patients to avoid alcohol during the course. Not because the medication reacts badly with it, but because alcohol is independently hard on the liver, and your liver has one job to do right now. Beyond that, you can work, travel around Thailand, and live normally throughout the 8 or 12 weeks.

After Treatment: Confirming the Cure

Finishing the tablets is not the final step. Twelve weeks after you complete the course, you come back for one more blood test called the SVR12. If the virus is undetectable at that point, you are cured. That is the clinical definition of cure, not just improvement, actual clearance of the virus from your body.

One thing I always make clear: being cured does not make you immune. Reinfection is possible if you are exposed again through blood-to-blood contact or sexual transmission. Safe sex practices remain relevant after treatment, not just before it.
