Gonorrhea is easy to dismiss and genuinely dangerous to ignore. The honest answer to "what happens if I leave it" is: a lot, and none of it is good. For some people, symptoms show up fast and are hard to miss. For others, the infection runs quietly for weeks or months while the damage builds. This article tells you exactly what that damage looks like and what to do if you are in Pattaya right now and need answers.
Gonorrhea Without Symptoms: How Long Can It Hide?
This is the part that catches people off guard. A large proportion of gonorrhea infections cause no symptoms at all, especially in women. Men are more likely to notice burning when they urinate or an unusual discharge, but even then, some feel nothing.
The infection does not pause while it waits for symptoms to appear. It keeps spreading through the tissues it has reached. Weeks can pass before anything shows up, and by then, early complications may already be developing.
Tourists are especially vulnerable to this pattern. You pick up an infection on a Tuesday in Pattaya, feel fine through the rest of the trip, fly home on Sunday, and only find out weeks later. By that point you may have also, unknowingly, passed it on. If you had any unprotected contact and there is any doubt, get tested before you leave.
What Untreated Gonorrhea Does to Men
The infection usually starts in the urethra, the tube that carries urine out of the body. Left alone, it travels upward. The epididymis, a small coiled tube behind each testicle, is a common next stop. That causes swelling, pain, and tenderness in the scrotum, and it can develop within days to weeks.
If it goes further, the infection can reach the prostate, causing pelvic pain, difficulty urinating, and fever. Over time, repeated or prolonged infection causes scarring inside the urethra. That scarring gradually narrows the tube, making urination painful and difficult, and reversing that damage is not simple.
Long-term untreated gonorrhea can also affect fertility. Scarring in the epididymis can block the passage of sperm, and that blockage does not always repair itself even after treatment.
What Untreated Gonorrhea Does to Women
Women face a more complicated picture because the infection has more places to go. From the cervix, it can travel up into the uterus, fallopian tubes, and ovaries. That condition is called pelvic inflammatory disease, or PID. PID can cause severe pelvic pain, fever, and discharge, but it can also develop with very mild symptoms or none at all.
Scarring of the fallopian tubes from PID is one of the leading preventable causes of female infertility. A scarred tube can also trap a fertilised egg before it reaches the uterus, causing an ectopic pregnancy, which is a medical emergency. The more times PID occurs, the higher that risk becomes.
There is a less well-known complication where the infection spreads to the tissue around the liver, causing upper right abdominal pain that gets mistaken for something else entirely. It is not common, but patients reach this stage simply because they waited too long.
Throat, Rectum, and Eye Infections
Gonorrhea does not only infect the genitals. Oral and anal sex can transmit it to the throat and rectum. Throat infections from gonorrhea are very often completely silent. You would not know you had it without a swab test. The same applies to rectal infections, which may cause discharge or discomfort but frequently cause nothing at all.
These sites matter for two reasons. First, they are a source of ongoing transmission even when genital symptoms are absent. Second, they will not clear without treatment targeted at that specific site, and a standard urine test will not pick them up. Eye infections from gonorrhea are rare in adults but serious. Direct contact with infected fluid can cause inflammation and corneal damage if not treated quickly.
Untreated Gonorrhea During Pregnancy
If you are pregnant and have gonorrhea, the stakes go up significantly. Untreated infection increases the risk of preterm birth and can pass to the baby during delivery. Newborns exposed in the birth canal can develop a serious eye infection that can cause blindness without immediate treatment. Testing in early pregnancy is standard care for good reason.
When Untreated Gonorrhea Reaches Your Blood and Joints
This is the most serious stage. When the bacteria enter the bloodstream, it is called disseminated gonococcal infection, or DGI. It is not the most common outcome, but it happens to people who assumed they could wait.
DGI causes fever, skin rashes, and joint pain. Bacterial infection inside a joint can develop rapidly and cause permanent damage if not treated urgently. In rarer cases, the bacteria reach the heart’s inner lining. That is a critical condition requiring hospital admission.

Why Antibiotic Resistance Makes This Worse in Thailand
This is something worth being direct about, because it is specific to this part of the world and most online articles miss it. Drug-resistant gonorrhea is a genuine and growing problem across Southeast Asia, including Thailand. Strains resistant to older antibiotics are now extremely common here, and ceftriaxone-resistant cases have been confirmed in Thailand in recent years.
The longer you wait, the more bacteria there are. More bacteria means a higher chance that a resistant strain takes hold, which makes the infection harder and more complicated to clear. Prompt treatment is not just about peace of mind. It is about keeping your treatment options straightforward.
Why Buying Antibiotics at a Pattaya Pharmacy Is a Gamble
Pharmacies in Pattaya are everywhere, and antibiotics are available without a prescription. It feels like the quickest fix. But self-treating gonorrhea in Thailand is genuinely dangerous, not just because of dosing errors.
