It reduces the risk of chlamydia, syphilis, and gonorrhea but does not protect against HIV or other viral infections. In Thailand it requires a prescription from a doctor. What Is Doxycycline PEP?
PEP stands for post-exposure prophylaxis. You take it after a potential exposure, not before. The dose is straightforward: 200mg as a single dose, ideally within 24 hours, and no later than 72 hours after the encounter. Earlier is always better.
Doxycycline works well against chlamydia and syphilis. Gonorrhea is more complicated. Thailand already has significant rates of tetracycline-resistant gonorrhea, so while doxy PEP may offer some partial protection, we do not rely on it for gonorrhea the way we do for the other two. That is worth knowing before you assume you are fully covered.
Who Should Take Doxycycline PEP in Pattaya?
The strongest evidence is for men who have sex with men and transgender women who have had a bacterial STI in the past year and are continuing to have condomless sex. Current CDC and BASHH guidelines both list this group as having the clearest case for doxy PEP.
What most clinics do not tell you is that both CDC and BASHH now allow for case-by-case consideration for cisgender women and heterosexual men too. The evidence is thinner for these groups, but the option is there. If you are a heterosexual tourist or expat who had a high-risk encounter, do not rule yourself out. Come in, talk it through, and make the call with a doctor who knows your situation.
Doxy PEP is not for casual use without a risk assessment, and it is not a routine substitute for condoms or STI screening. That is why it requires a prescription, not a pharmacy shelf.
The 72-Hour Window
Seventy-two hours sounds like plenty of time. It is not, especially on holiday. A lot of tourists we see have already lost 24 to 36 hours second-guessing whether they really need to come in. By the time they read a few forums, ask a friend, and finally walk through the door, the window is closing.
If your exposure happened last night, come in today. Not tomorrow. Today. Within 24 hours is ideal. Between 24 and 72 hours is still worthwhile. After 72 hours, doxy PEP is no longer recommended. Doxy PEP Is Not HIV PEP
This comes up constantly and it matters. Doxycycline PEP does not prevent HIV. It does not prevent herpes, HPV, or mpox. It only targets bacterial infections.
If HIV transmission was a realistic risk, you need HIV PEP, which is a separate antiretroviral regimen that also has a 72-hour window and runs for 28 days. What Happens After You Take Doxy PEP
Taking doxycycline PEP does not end the process. We recommend a full STI screen at the time of your consultation to set a baseline, and again at six weeks and three months after the exposure. If you are using doxy PEP on a recurring basis, CDC and BASHH guidelines both recommend testing every three to six months. That schedule is not optional. It is how we catch anything that slipped through.
