Doxy-PEP is a single 200mg dose of doxycycline taken within 72 hours of condomless sex. It reduces the risk of chlamydia and syphilis by more than 70%, and gonorrhea risk by roughly half. It does not protect against HIV, herpes, or HPV. A doctor needs to confirm it is right for you before you take it.
So last night did not go exactly as planned. Maybe you skipped the condom, maybe things just moved fast, and now you are sitting in your hotel room somewhere in Pattaya at 7am running a search on your phone. There is something you can do. It is called doxy-PEP, and the window you have right now is real. Think of it as a morning-after option for bacterial STIs. Not a guarantee, but a meaningful layer of protection you can still take after the fact.
Doxy-PEP stands for doxycycline post-exposure prophylaxis. It is a single antibiotic dose taken after unprotected sex to lower your chances of getting chlamydia, syphilis, or gonorrhea. It is not a replacement for condoms or regular STI screening. But if you have had an unplanned exposure, knowing about it before that 72-hour window closes matters.
What Doxy-PEP Protects Against — and What It Does Not
Doxy-PEP works against three bacterial STIs: chlamydia, syphilis, and gonorrhea. The evidence for chlamydia and syphilis is strong, with clinical trial data showing a risk reduction of more than 70%. For gonorrhea, protection is closer to 50%, and that figure may shift as antibiotic resistance grows.
Here is the part that matters most. Doxy-PEP does not protect against HIV. It does not cover herpes, HPV, or mpox. How to Take Doxy-PEP — Timing and Dose
The dose is 200mg of doxycycline, taken once after the exposure. That is typically two standard 100mg tablets taken together. You do not take it before sex, and it is not a daily pill.
Timing matters more than most people expect. Within 24 hours is ideal. Within 72 hours is the cutoff. After that, the evidence for benefit drops off, and a prescription is no longer appropriate for that exposure.
Take it with a proper meal. Doxycycline on an empty stomach is one of the more reliable ways to spend your morning feeling sick. If you are already stressed about last night, that is not something you need on top of it.
Side Effects, Safety, and the Pattaya Sun Warning
The two most common side effects are nausea and photosensitivity. Nausea is manageable with food. Photosensitivity is the one that catches people off guard here.
Doxycycline makes your skin much more sensitive to UV. In a beach city with this kind of sun, that is not a small thing. If you are planning a day at Jomtien or heading to one of the beach clubs on Pratumnak, use a high-SPF sunscreen, cover up more than usual, and limit direct sun exposure for at least the first 24 hours after taking the dose. What would normally be a short time outside can result in a serious burn.
Doxycycline can also affect your gut. A single dose is unlikely to cause lasting disruption, but if you are already dealing with traveler’s diarrhea or an unhappy stomach, flag that when you come in. We can talk through timing or suggest something to support recovery afterward.
The Antibiotic Resistance Question
This part of the doxy-PEP conversation does not always get said plainly enough. Doxycycline is a tetracycline antibiotic, and gonorrhea has a history of developing resistance to antibiotics over time. Research published in 2024 and 2025 flagged rising tetracycline resistance in gonorrhea strains across parts of Southeast Asia, which matters in a city like Pattaya.
What this means practically is that doxy-PEP’s protection against gonorrhea may become less reliable over time. It is not a reason to skip it when it is appropriate, but it is a reason to use it thoughtfully rather than routinely, to still use condoms where possible, and to get tested even after taking a dose.
This is also why picking up doxycycline at a Thai pharmacy without a consultation is not the right approach. Using it without guidance contributes to resistance and leaves gaps in your care. No contraindication check, no follow-up plan, no testing schedule. A proper consultation is part of how this works safely.
Who Should Not Take Doxy-PEP
Doxy-PEP is not for everyone. If you are already taking doxycycline or another tetracycline antibiotic daily, adding a dose on top should not happen without medical guidance.
Pregnancy is a significant caution. Doxycycline can affect fetal bone and tooth development and is generally avoided during pregnancy. If there is any chance you are pregnant, tell the doctor before a prescription is written.
Drug interactions are worth checking. Some seizure medications, blood thinners, and certain antacids can reduce how well doxycycline works or increase side effects. A brief health history review at your consultation catches these before they become problems.
Doxy-PEP vs PrEP — Not the Same Thing
These two terms get mixed up constantly. PrEP is a daily tablet taken before any exposure to prevent HIV. It is for HIV only. Doxy-PEP is taken after exposure to reduce the risk of bacterial STIs. The two can be used together, and in fact the clinical trials that established doxy-PEP’s effectiveness were largely done in people already on PrEP.
If you are on PrEP and had unprotected sex, doxy-PEP can cover the bacterial side. If you are not on PrEP and had a potential HIV exposure, doxy-PEP alone is not enough. Why Regular STI Testing Still Matters
Doxy-PEP is not a substitute for regular STI screening. CDC guidelines recommend HIV and STI testing every three to six months for anyone using doxy-PEP with any regularity. That exists because doxy-PEP reduces risk, not eliminates it. Some infections, particularly gonorrhea, may still get through.
